Case Management
Welcome to the Case Management Department. Case Management is part of DotHouse Health's commitment to complete and comprehensive care. Case managers are able to connect patients to social services and clinical support resources available in the health center as well as with other community based organizations and agencies.
We feel strongly that case management is critical in helping patients address pressing needs that occur outside the exam room.
What can Case Managers do?
Case management services include:
- Connecting patients to health education and support classes, clothing and basic baby items.
- Addressing food security issues through our food pantry, our farmer’s market, and $2 food bag sites.
- Helping to apply for SNAP Benefits.
- Offering referrals to credit and financial counseling, education, job training programs and employment.
- Utilizing existing partnerships with community based organizations to advocate for patients’ rights and responsibilities (i.e.; Ropes & Gray pro bono legal services, Massachusetts Coalition for the Homeless, Project Bread, ABCD Fuel Assistance).
- Navigating families through early intervention screening and individual educational plans within the Boston Public School system.
- Linking families to support services including childcare and parenting workshops.

FREQUENTLY ASKED QUESTIONS
How do I schedule an appointment with a case manager?
Patients are referred by their primary care providers for appointments with case managers. You must have a primary care doctor at DotHouse Health to receive services from a case manager. If you walk-in to meet a case manager, we will register you as a patient and connect you to our healthcare services.
Can a case manager find me housing?
We cannot directly connect you with housing. However, our case managers can connect you to housing resources in Boston including the Boston Housing Authority, ABCD Housing and Homeless Prevention Program, Project HOPE, Quincy Housing Authority and Brookline Housing Authority.
How can I apply for benefits?
We can only complete your application for SNAP benefits (food stamps). We are able to assist with the application process for SSI (Supplemental Security Income) and EAEDC (Emergency Aid to Elders, Disabled and Children). However, you will need to complete these applications individually at the SSI and EAEDC offices.
Do I need to have insurance to see a case manager?
No. Insurance is not necessary to meet with a case manager.
